The guided interview and the living clinical record: the heart of the plugin, always on.

What it's for

Claude leads an interview in small steps, across many chats, to rebuild with you your clinical history and the way you function today. Everything that comes up goes into the clinical record, a file in your folder that you can read and correct whenever you like.

  • Facts, memories and hypotheses stay separate. Every piece of information has its source: a document [D], your memory [R], someone else's account [A], a hypothesis to check [I], something to find [?].
  • Hypotheses stay questions. Claude doesn't diagnose: hypotheses become questions for the people who follow you.
  • Real problems, practical strategies. With “problem:” you work through a real situation and agree on a strategy, with a date to check whether it works.
  • Material for professionals. A one-page summary for a new doctor, or questions for someone who knows you well.

let's start

Claude

Hi, I'm Claude. In this space I work in the role of a neuropsychologist, but I remain an AI: I don't diagnose and I don't replace the people who follow you.

We'll rebuild your story together, in small steps, and everything will go into a record in your folder. To stop, just type “pause”.

To begin: what would you like me to call you? Would you prefer Italian or English?

You

Marta, English

Claude

Nice to meet you, Marta. What is the neurological condition, in the words of your reports or in your own? “I don't know yet” is fine too.

You

Brain injury, after a bike accident in 2021

… a few more questions: who follows you, which tools you want, how you'd like me to talk to you …
Claude

One last thing: would you like me to update your record on my own at the end of every chat, or would you rather ask with “update record”?

You

I'll ask

Claude

All right. I've created your record and your command guide.

⚠️ At the end of a chat: I don't notice when you close it. Before closing, type “update record”. In your next chats, type “let's start” and we'll pick up from here.

Claude

Hi Marta. Last time we talked about work, and you tried the aisle-order list at the supermarket: the check was due today. Shall we start there, or is there something more urgent?

You

There. It went better, but I can't always go on Tuesday mornings.

Claude

Good that it went better. Let's keep the aisle-order list. As for timing, is there another quiet moment that suits you?

You

Thursday evenings, after 8 pm

problem: · Simulation with made-up data. Claude's real answers differ every time.

You

problem: when I go to the supermarket I always forget half the things, even with a list

Claude

Let's start from the last time it happened. How was the list written, where did you keep it, what happened once you were inside?

You

Saturday. The list was on my phone, in no particular order. It was crowded with loud music, I stopped looking at it after the fruit aisle.

Claude

Thank you, that's a very clear example. Your record already says that noisy places tire you quickly: the list was there, but the setting drained the energy you needed to follow it.

I suggest two things to try, no more:

  • write the list in aisle order, so you follow it as you walk;
  • go at a quiet time, for example Tuesday morning.
Shall we try for two weeks? On the 28th you tell me how it went.

You

Yes, let's try

I found the MRI report from 2022. It says “sequelae of right frontal contusion”.

Claude

Thank you. In plain words: the MRI saw the marks left by the blow on the right side of the forehead, an area that helps with organising and regulating emotions. It's what the report says, not a new diagnosis.

Claude

I've added to your record:

  • section 6, tests: 2022 MRI, “sequelae of right frontal contusion” [D, 2022 report];
  • in the session log: “read the MRI report”.

Claude

Before I prepare it: is there anything you'd rather leave out?

You

Leave out the part about work

Claude

Here's the draft, one page: reason for the summary, history of the injury with tests and dates, what works and what's hard today with an example for each area, strategies in use, open questions for the neurologist.

At the bottom it says: “Summary prepared with an AI assistant from the person's accounts and the documents listed. It is not a clinical assessment.” Is this all right?

Yes, that's fine

questions for Luca

Claude

First of all: do you really want to involve Luca? And about which period?

You

Yes, he's my husband. The first months after the accident, which I don't remember well.

Claude

Here are six questions for Luca. Two examples:

  • “Do you remember a time, in those months, when I seemed different from before?”
  • “What did you see me most at ease doing?”
And two lines to explain why you're asking: “I'm rebuilding the months after the accident and there are things I don't remember. Will you help me?”

A few days later
You

Luca says that in those months I often asked the same questions again

  • clinical-record.md: your story, how you function today, strategies, hypotheses and the session log. Section 0 holds your settings and preferences, such as the topics you don't want to touch.

  • backup/: up to three backups of your record, made before each tidy-up.
